We are very disappointed with your service. Our clients are facing a lot of problems while submitting the form. Due to this issue we are losing our customers, they all are complaining about unsuccessful form submission.
Here I've also attached a screenshot sent by our customer. When he was trying to submit the form after filling up all the details and uploading the images an error window appeared.
You are requested to kindly solve this issue asap otherwise we need to move on to typeform service.

I cloned your form but it loads fine on my end. I was able to submit your from without an issue. I noticed that the form has a lot of upload fields that maybe have caused the form to get this error when the user upload lots of images with a bad internet connection on their end.
Can you please ask the users of the form to submit your form again? Make sure that the required fields are all filled by the users.
If issue still persists, please try to use different browsers. We recommend users to use Chrome or Firefox to submit your form. I already clear your form's cache, please also try to clear your browsers cache and see if this works.
