You can check out the website. www.ezcaregiver.weebly.com. The patient will not be able to type an email into each form. It has to automatically be there. Can you do this with Jotform?

I am not sure if I understood your question correctly. If you want to create login form, that is not possible because we do not allow this type of forms.

Or you want to prepopulate email field? If so you can set default value inside of email field and set field property to be read only. This way your users will not be able to change it, but email address will always be the same.
