Form is Disabled

We've tried to create a form and it's been disabled (along with all of our other forms).

I suspect it's because of the word "password".

The form has been changed now, but how can I get the forms re-enabled?

Also, can we use any of these terms: "passphrase", "passcode", "passwd", "PW"?

You can not use any of those words. I have inspected your form and I saw that your form also has username field.
