Are there designers that I can pay to create my form?

I would like to hire someone to create several different forms... Is there someone you recommend that I can speak with on the phone?

There is no specific person I can suggest, but I would recommend checking freelance websites such as Upwork.com or Freelancer.com. There are designers there who can help you create the forms based on your requirement.
