all of my forms are disabled? Why?
Can you please enable them?

Your account was auto suspend by the Anti-phishing tool that's why your forms has been disabled. I went over your forms and I noticed that the form (http://www.jotform.us/form/70401006042132) is asking for a credit card details. Please note that asking for such details to the users is against the Term of Use.
I removed now the said fields and make your account active again. Please refrain from doing this so that your will not be detected. If you would like your form to ask payments from the users, you may consider to use one of the Payment Processing Apps available in JotForm.
