Both of my forms are disabled again when I try to preview them. I have checked in the preferences for each form and they say that they are enabled.

I have inspected your forms and I did not found any fields that are violating our Terms of use. I also saw that this is the second time this has happened and that my colleage whitelisted your other form https://www.jotform.com/answers/1052293.
