Unable to integrate with my box account.

    Unable to authenticate into box.  Cleared browser Cache & form Cache.

    login to box and it says username:  travis.fox@charter.com

    upon authenticate it changes usernname to jtravis.fox@charter.com

    Help??

    I am not sure if I have understood your issue correctly. Where do you actually see the username changed upon authentication?     I did try to integrate my own form with my box account and I am not able to replicate the issue you are having. I was able to integrated successfully. Please check the screenshot below:
