I have just tested our DocuSign widget with our Form Builder version 4, and it seems to be working fine. Screenshot from adding DocuSign in V4:

In case that your DocuSign widget is not working correctly, there may have been some kind of corruption.
Please try running its wizard again and click on Update Widget, and should the problem persist after that, please try removing the faulty widget and then re-adding DocuSign into your form again. Thank you.
