Form submission data not showing on email notification received when form is submitted

i tried to fill in the form once it was embedded on my website, but when i received the email it only showed the title but no information that i had filled in? 

It is highly possible that the Field Name Tags in the email notification body is not updated.

You can get the updated Field Name Tag(s) from the Form Fields drop-down in the email content toolbars.

Or, you can also delete the existing Email Notification then add a new one.
