How can I add a logo to the form?

You can use the "Image" tool to insert a logo. In Form Builder V4, click the "Add Form Element" button. You'll see the "Image" tool under the "Basic" tab. Click or drag it to the form.

In the "Image Properties" click the "Choose a file" button and then click the "Upload File" button.

When you're done, click the "Save" button.
