Form not found. This form is disabled.

I use JotForm, to receive subscriptions to my class, since I started this. And I use, the same form as ever. I just duplicate it, and change the dates. What could possibly had happened differently this time?

I have checked and your account is autosuspended by our phishing system. Inside of your form Inscrição Curso Revit 2017 (13) you are asking for user login credential (username- Nome de usuário AVA) and that and that is in violation of JotForm Terms of Use.

Phishing - You agree that Company may terminate your JotForm Service immediately If a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately. 

I have removed mentioned field from your forma and I have activated your account. I would suggest you to remove same field from your other forms as well. Please do not create forms that are violating our terms of use to prevent that your account is suspended again.
