Why can't my form be filled in?

I uploaded my form to my site. I went on my site to test the form and see what would happen if someone sent the form. 

I could not fill in the text fields on my form. Why is this?

Also the theme I choose I'm unable to see it as a whole dog. Why is that?

Your form can be filled, but text color is white and because of that is not visible. 

Same issue can be replicated on standalone form http://www.jotform.us/form/70397475847169.

You need to change the font color of your text. You can do that inside of Form Designer.

It seems also that your theme was not applied. Please try to select your theme again. You can select some other theme and than select again Pet Lover theme.
