I reported same problem yesterday and the problem was resolved this morning. I made a small change to the form. As a result, the forms again disappeared? I do not know what it is.

Your Form Status is set to Disable on date. Please enable your Form Status so you can use your form properly. Here's how:
Open your form and go to the Settings page. Select the Form Settings tab and Enable the Form Status.
