Form embedded using lightbox opening blank.

We have used the lightbox a lot and all our other pages work well.  We have created a slightly longer lightbox and it doesn't seem to function well.  Take a look at :

Click on either the link in the main body or the first link under HOW.

 

The light box displays but doesn't populate - when you click X it then displays the info.

Apologies for any inconvenience. Upon checking your website and clicking the link, I was able to reproduce the issue you are having. I will further investigate the issue and get back to you as soon as I found a resolution for your concern.

After further investigating, I found out that when you click any of the links, two forms will be opened and one of which is blank.

The probable reason for this is there are two same lightbox scripts added on your form and when you click any of the links, it will call both scripts.

What I suggest you do is remove one of the scripts which will have you two links only calling one script.
