We've tested this further from our end where we found we were able to view your forms normally in the My Forms page so we've cleared your browser cache to see if that helps to resolve it.
Please try this again on your end by fully logging out, log back into your account, and look at the My Forms page to check it.
If you notice there's still a problem do let us know so we can do some further investigating.
