I have tried submitting your JotForm http://www.jotformpro.com/form/63558785813975 using Check payment and was able to submit the form without any issue?

That doesn't work on my Mac at all. Firefox hangs forever regardless of payment method. This happened all of last year (where I finally just gave up on it). It continues this year. I have to get people to use alternate browsers.
There are no error messages, the browser sits with the Submit Form button saying Please wait...
I just did this again today at 1301Z coming from 188.8.131.52. You should be able to check webserver logs for those, although you'll also see me coming from Chrome around the same time (to draft this message).
After 10 minutes, a single 222KB file is still providing the Please wait... button.

I have tried submitting the form again using Firefox and on Mac OS was able to submit without any issue at my end.
I was not able to replicate the issue at my end. Please let us know if the form is embedded on any of your web pages and experiencing the issue. If so, please provide us with the web site URL so that we can take a look and provide you with necessary assistance.

I am still have the issue, just got a confirmation from another user that it failed for them on Firefox. I cannot tell whether they are Mac or Windows.
I'm running the same version of Firefox. Running on Maverick's last year, and El Capitan and Sierra this year (failed before and after the upgrade).
What did the server logs show? Are you seeing the attempts to upload that do not succeed?

I see that your form has been disabled already. Though when I tried to clone your form, I've tried a number of times to replicate the submission error. Unfortunately I was not able to.
Do you have the specific error captured or would you recall the specific error that appeared?

As I said before, no error is ever presented. The upload hangs in perpetuity. The Submit Form button changes state to Please wait... and nothing else happens.

The form given in this thread - http://www.jotformpro.com/form/63558785813975 - is no longer active. Maybe I am missing something but can you please give us the link of the new form you are working on? If it is embedded to a webpage, please let us know the URL as well.

It is an annual event and is closed again for the year. I'll probably just use an alternative platform next year, this has been an unresolved problem for more than a year.

We apologize for any inconvenience caused. I understand that the form https://www.jotformpro.com/form/63558785813975 was already disabled so we cannot do a test submission for it anymore.
But we can just clone the form and use the clone version to further investigate the issue.
I made a test using a cloned form https://form.jotform.com/70486888757984 but I also could not reproduce the issue as well.
Unfortunately our option to troubleshoot the issue was being limited at this time since we could not find enough information about it. Since we could not produce it on our end, we could not provide the necessary data to our dev support so they can take a look and fix whatever the bug/glitch was.
If you are able to get a hand on more information about the user experience that encounter the issue please share to us and we will look into it more.
