The name of the form is not displaying correctly in Facebook post

When i try to share the form the name of the form is wrong and I cannot find out where i change it.

 

Generally, Facebook should be displaying the page title of the form. Could you try changing the page title to a different one and see if a new post in the Facebook is displaying the changed title?

I was able to replicate the issue. Here's a screenshot:

My colleague is correct when he said that Facebook is using the Page Title when you sharing a link. I have scraped the new information and it is now fixed. Here's a screenshot:

The problem remains though. I have now changed it to the correct tex "Outcraft Crew". Could you please correct it?
