Can JotForm do what we need?

    Hi

    Here's the short version...

    We are a smallish UK nonprofit that promotes walking as a healthy and sustainable means of transport.

    We would like to create a tool which will allow people to rate a walk they do every day.

    - We would need users to enter personal details - first name, last name, UK postcode and email address, plus a dropdown question which would categorise.

    - The rating aspect would ask users to provide a score out of ten for 7-12 different criteria.

    - At the end we would want the user to see their total score and offer conditional content based on the final score (we could band these to make this simple)

    - We would then need to receive all of the data as a xl or csv file.

    I do believe our forms would be able to meet your requirements.  I will run down your questions in order.

    - We would need users to enter personal details - first name, last name, UK postcode and email address, plus a dropdown question which would categorize.

    We have basic fields that can be added for each of these:

    For this, you could use an input table field:

    - At the end we would want the user to see their total score and offer conditional content based on the final score (we could band these to make this simple)

    You can perform calculations on the entries in your table to provide a final score:
