    You may try changing the URL in the script from https://form.jotformeu.com/jsform/63487091483363 to https://form.jotform.com/jsform/63487091483363 so the embed line becomes: <script type="text/javascript" src="https://form.jotform.com/jsform/63487091483363"></script>
