Survey rating trigger image

I am building a review collecting form and would like the report to be embedded on my website. I am using the survey rating field to have the patients rate my business but I would like a survey image to show in the report according to their choice: 5 star rating would trigger a 5 star image.  I have the images in the image picker widget right now and they do show on the report but I would rather use the simple star scale.

I did check your report and you seems to have configured it correctly to display image instead of link. Please check the screenshot below:

The stars you see on the reports are from the image picker. 
What I would like if to use the star rating option and have that trigger the star images on the report. 

I do not want to have all 4 images showing, I just want one star rating.

So is there an condition that would load an image into a field when someone chooses a rating?
So if they pick 5 stars, the 5 star image would appear in a hidden box that showed on the report but the actual #5 does not. 

We would like to apologize for any inconvenience, unfortunately, it is not possible to pass a value from a field via conditions to widgets, that is why creating a condition to select a certain image on the Image Picker widget based on the selected star in the Star Rating field will not work.

If I load 4 images and make them hidden, and I set a condition that says if star rating is 5 then SHOW 5 star image, would that work? 

Yes this is possible to achieve using star rating field and condition logic.
