How do I fix the errors in the form for this website?

The form is one I have for mobile and desktop versions.  The mobile fills in correctly and goes to the Thankyou page as set.  The desktop has errors and will not post.

I have tested your embedded form and I was able to replicate mentioned issue. After that I have tested your standalone form and I submitted the form without any issues. 

After that I have inspected your webpage and I saw that you have embedded your form with full source code. To resolve your issue you will need to re-embed your form. Please note that each time you modify your form you will need to re-embed your form, if you are using full source code. 

If you do not want to re-embed your form, each time form is modified, then I would suggest you to use iFrame code. Inside of this guide you can find how to get iFrame code of your form:
