"Balance Point New Hire Setup" on my account. Someone tried to submit the form using an iOS device and received error on submission, but I was able to submit the form using Firefox on a Windows PC. Shouldn't the form work from and iOS device? It's not a complicated form, just a few required fields.Page URL:

The forms should work on all devices, doesn't matter the operating system or the Browser. I checked your form from iPhone simulator and it worked fine:
It would be great if you would have any error message or anything they saw so we can investigate further. Or maybe it was a temporary glitch with phone.
