I have a client with potentiality dozens and dozens of business cards design. What I would like to do is upload a PDF (or JPG, PNG or whatever) and place fillable fields over the card so when the customer fills out the card they will see what it looks like.
Attached is a screenshot to hopefully help explain. The yellow highlighted boxes would be fillable fields.
Ultimately - if the form could be saved as a pdf when its submits - that would be awesome.

I am not sure if I understand your query correctly. Do you mean to have the form filled information to be displayed in the Business card format provided and can be downloaded as a PDF file? If so, you may have a form created with the fields that are required to be filled on the Business card, and customize the PDF report with the business card format.
This PDF report can be attached to the auto-responder/notification so that the submitting person can receive as an attachment.

What I am looking for is to display the actual card as the form. They will fill in the areas the need to ans will see what they are getting as the fill in the form.
So can I put the card as a background image and fields over top?
I also don't see where in the form your idea of PDF reporting and attachments is can you send a link?

Yes, it's possible. You just need to style the form to look like the card by custom CSS codes. We can help you with this. Can you send us the logo? The website in the card is using a different logo.
For the Customize PDF option, you have to access the form submissions page: https://www.jotform.com/help/269-How-to-view-Submissions.
For the Attach PDF, it's in the email notification settings.

Making the form and the PDF look exactly like a business card may take a bit more time than I can commit at the moment. I have started a form but it proving to be a bit more work to get things lined up correctly than I had anticipated. If the support forum slows down a bit, I will take another try.
In the mean time, if you'd like you see if you can make the changes on your end, we will be happy to help with whatever problems you run in to.
