How to add products with images and prices to the form

I want to make a form for customers that shows options and prices.  I would like to include photos of the options along with the price for that option.  Then, the customer can make a choice and click the radio button.  It would be great if the price for the item along with the option price could be added to a total.

I realize that this is a much expanded version of what you have now  Maybe something for later.

Please try a Purchase Order tool from Payments section. You can add your products with images and prices, the total amount will be also automatically calculated.
