The PDF form unfortunately has many limitations over the online web-form, as described in the following guide:

    https://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator

Unlike our web forms, the PDF design cannot be as easily adjusted and customized. The form will not look as good when it is accessed as a PDF form, as it will look when it is accessed online. However, if you let us know the form you are working on, we'll be glad to take a closer look and see if there is anything that can be done on that specific form to improve its appearance as PDF.

    Thank you!

We are sorry for the inconvenience this may have caused. I have cloned your forms to my account and tried creating PDF forms. Unfortunately, I am also able to create the PDF forms with the same issues. This is due to the headers and text fields on the form which are being converted as images and the length of the form might also be causing the issue.

As already mentioned in the guide, there are still improvements to be made on the PDF form creator. I have also tried creating a clone of the form without any styles and the issue still exists due to the limitations of headers and text fields on the PDF creator at this moment.

    Thanks!
