Hi I cloned an existing form to a new form. When I open the new form all the current information is there and I can submit the form. However, when I get the new submission response it has the question data from the old cloned form! HELP

I have tested your form and found that it sends the correct data, if you check the submissions page you can see the correct submitted info.
However, may be that the email notification is still the one from the cloned form, so I would suggest you to delete the current emails and add a new one, this way you get the fresh data from the new form.
