Why is the account suspended?

    Your account was suspended due to suspicious fields on the form. Please note that collecting login credentials such as passwords is strictly prohibited in Jotform. Our phishing detector will automatically suspend your account if a password field is found in one of your forms.

    To resolve the issue, I have removed the password field on your form and reactivated your account. Your form should be active now. Please refrain from adding password fields to your form again.
