It is not generating a proper pdf copy of the form

I have attached a copy of what I get when I tried to generate a pdf of one of my forms. I was really enjoying this web based application until I've started to use the conditions and it's not working. I have attached a copy of the pdf output.

Are you referring to the converted fillable PDF version of the online form? Please note that there are a couple of functionality in the web form that will not work on PDF. To learn more about this, please do check this guide: https://www.jotform.com/help/312-JotForm-Fillable-PDF-Creator-With-All-Points. An example of this are the conditional logic, they will not work in PDF. 

Let us know if I misunderstood your concern. If the issue on PDF and conditions are separate, please do open a separate thread about the conditions for us to discuss it better and to avoid confusion.
