Why does it keep saying form disabled when I am ready to preview my form?

I've enabled my form, I've edited and done everything I can think of. Why am I not able to preview my form because it has been disabled? I need help. I am extremely happy, visually with the form I have created. I am upset that it is not working. Please help.
