How to add an additional optional POP up Form?

We sell a product that requires somewhat detailed information if we add a dependent.

I am using this form for in house sales reps.

For example, when they fill out the form for Dad, then it's just the main form.

But let's say Dad has two kids, then I would need additional fields to pop up at the bottom of the form.

If you look at my form you'll see member details, so let's say select 03, then at the bottom more fields will be available for the rep to fill out.

What would you recommend?
