How do I pull a submission report that shows the last 4 digits of a credit card for a payment form?

This will not be possible to achieve since you cannot ask for this info in a plain text, which would be required to get the info displayed in a report.

If you need to request payment info in your form you must use one of our existing payment integrations, since JotForm does not process the payments it goes against our terms to ask for payment info without a payment integration and your account will be suspended.

You can read more about it on our terms page: http://www.jotform.com/terms/

You will find more info about payments forms on this link: https://www.jotform.com/help/chapter-7-Payment+Forms

And also, if you would need to capture the payment info only without charging your customer, you could do it with the integrations that allow this feature, here's a link with info about it: https://www.jotform.com/help/292-How-to-Enable-Payment-Authorization

    Hope this helps.

Here's our connundrum: JotForm doesn't capture the last 4 digits of a donor's credit card (as our previous forms product did) and Square (our payment gateway) doesn't seem to capture the name of the form our donor has used to make a donation or register for an event.

What are your suggestions? Is there a way to pass the name of the form to our gateway using a JotForm API?

    Again, thanks for your help.

Unfortunately, will not be possible to pass a custom field to Square, the integration does not allow to map it, though you can get it with the JotForm API: http://api.jotform.com/docs/#form-id

There're only some other integrations that will allow you to pass a custom field through the integration like PayPal for example.
