I need to add a Malpractice Document that needs to be signed. Currently, it's manually added into my form at the end but all I see is the patient's signature which is not enough for the actual document to be in effect. For example all you see is
Patient Signature: (Please type your full name below) David M Horn Date: 1/11/2016 Patient Signature: (Please type your full name below) David M Horn Date: 1/11/2016 PATIENT SIGNATURE X David M Horn DATE 1/11/2016 PATIENT SIGNATURE X David M Horn DATE 1/11/2016 OFFICE SIGNATURE X -

karenacucareReplied on February 23, 2017 at 3:46 PMIT’s not that I need it to be an actual signature it that i need to be able to see the entire document, not just the form sent to me with the patient’s signature. You can’t tell what was singed. hope you understand
