My form is titled "2017-03-11 Shushan Purim the Musical" and located here:
This form has a radio button that let's people choose to pay for their reservations "online" via credit card / PayPal OR "offline" via check / cash.
EVERY TIME someone chooses the "offline" option the form duplicates the entry when the user clicks submit. The duplications are only ONE second apart making me think that this must be some sort of system error.
This does NOT happen when a user chooses the "online" option. It only happens when they select "offline."
There are no "mailing" conditions in the conditions list. In fact, I kept the conditions list very simple for this form with only a few conditions to determine when to display or hide sections of the form. The emails that are triggered by the form are exactly the same emails regardless of what choice the user makes.
Please help. We would like to go live with this form ASAP.

I have checked your form and was able to replicate the problem on my end, I can also replicate it on a cloned form of yours as well as a bran new form with payment info and method selection only.
After investigating the issue I can see it's being caused due to the hidden payment field under the collapse tool, currently you're hiding/showing the payment section with conditions, seems like this happens when you conditionally hide the collapse tool where the payment field is placed in.
A way that I found to resolve the issue is to include the payment field in the condition that hides the collapse.
This will help to stop duplicated submissions at the moment, I will also submit this to our second level so our developers can investigate and resolve it, we will let you know as soon as we get any update regarding this.

GREAT! Adding the extra "hide multiple" condition and including the section AND the payment integration field worked. Although, it does seem like this is a bug because that field is ALREADY hidden in the collapsed section. But, as long as it now works...

Glad to know this worked for you.
As per my investigation, I was able to replicate it hiding the collapse only, seems like the hide functionality is not working the same in the collapse tool as well as the condition, this is why you can see the problem does not happen if you hide the payment field with a condition.
However, this will be further investigated by our developers and we will update you about any new regarding this.
