Is it possible to link an existing contact form to my JotForm account

I have a contact us form embedded in the website that the site developer created, but I would like any information submitted via the form to link to my JotForm inbox and send an email notification. 

The form will need to be move from the other account to yours, but the request must be opened from the form's owner. 

We need to get the form's ID or name as well as the target account.
