Why are my forms disabled when I embed them on Weebly?

I created two forms and every time I try to use them, they say they are disabled. Please help.

Your two forms are asking for credit card details using our plain textbox field. Please note that we do not allow this type of form. We consider this as phishing which is against our Terms of Use (http://www.jotform.com/terms/). 

I have reactivated your account, but I have deleted the said fields. Please refrain from adding the fields again. If you want to collect payment at a later date, you can use our payment integration such as Paypal. You can enable authorization for charging users later on:
