Can a printable version of the data I collect be created?

I have set up a form based off of a form given to our animal shelter by a veterinary clinic which is based in Microsoft Pages.  They are looking to have the very same appearance to the printed out form I give to them.  I have set up a form on your site for collecting data, which will hopefully be done through our online website to cut down some of the workload for me as a volunteer.  However, I am unsure how to get the appearance they are seeking using the suggested integration programs.  Can you please assist?
