I keep getting a "form disabled" message whenever I preview or publish any of my old forms. I recently got locked out of my account and reset password and changed email (not sure if that's related).

You've been auto suspended because of the credit card fields in your forms. You've posted a thread yesterday (https://www.jotform.com/answers/1076479). My colleague answered you there, please check and send your reply there.
