Incorrect Results on Quiz Form

I'm getting incorrect results when using the Quiz Form app on the url below.

It doesn't show which answers I got correct and if I deselect "Show Results to user after submission” the attached screen capture appears which is clearly incorrect as there are 10 questions and not 5. 

I tested your quiz form and I was presented with the correct totals:

Is this not what is being shown on your end?  As far as I can tell, your quiz form is working as it should.

Also, the results are incorrect on both and it shows 0/11 when there’s only 10 questions.

The quiz app is counting the total field as a question.  If you want to use a total field in the form, assign calculation values to your options:

Then, make sure all your fields are being included in the calculation:

Currently, only 5 of your fields are included in the calculation.
