extra blank page in between my form

Please take a look at this form: https://form.jotform.com/70536942081153

and when you select any choices from the 1st dropdown menu, there is an extra "blank" page shows up in-between the pages.... and I can't figure out why where that blank page is coming from because it didn't show up on my form in the "building mode".

The second page appears to be blank when you choose Policy 103 (new) in the first page.

 

Please note that you have the following conditions when Policy 103 (new) is not selected.

 

Therefore, when Policy 103 (new) is selected, the fields in page 2 are hidden (the reverse of the above condition).

 

You may want to consider skipping page 2 when Policy 103 (new) is selected as shown below.

One more question:
How can I add a hyperlink within the text on Jot form?
For example, on this jot form:
https://form.jotform.com/70574182731153
I want to include a link to this web page:
http://district279.org/about-us/school-board/policies-and-procedures
But I couldn’t find a way to list hyperlink so I can embed the whole page (using PDF option) which I don’t like how it looks.
