I created the form below but it does not show as items selected in the email I receive.

I have created the form below but when I send the request it only show the amount not the item and the quantity. Please advise

It seems that there is an issue with the notification created on the form. Could you try creating a new notification and see if that works without making any changes? Please refer to the guide below that can help you with creating a new notification for your JotForm.
