greetings. We upgraded our account recently and our form is still not active. Please help.Page URL:

I have checked and your account is active.
Your form is disabled, because on Form settings page of Form Status is set to Disabled.
To resolve your issue you need to enable your form. Simply change Form Status to Enabled.
