Agents are getting a error message saying "Upload error File could not be uploaded for some reason.
Please go back and fix the problems."
When they click go back they are returned to the original blank form.Page URL:

This should be fixed by now. I am able to send a test submission to your JotForm successfully. Please check and let us know if you still experience any issues with the file upload on your forms. We will be happy to take a look again.
