My Forms Disappeared

I clicked on learn about 4.0, and the forms that I had made and saved are no longer there.  All that is there is a empty form template titled heading.

I had an adoption form, sponsorship form, fun run form, volunteer form....all gone. And I didn't hit delete

They show on the website and the facebook page, but simply because they are embedded with the old code that is no longer there.

These are the embed codes for the forms that are no longer in my account. How do I get them back.

Forms that you provided are created on account with username handsofhopedana. You are a sub account user of that account and you need to click on Shared with you folder to view them.
