jot form not showing values in email

I have checked your form, and it seems to be working fine - the values are correctly being replaced with what your users fill out on the form.
Based on your screenshot, I see that it actually refers to a Test Email. Please keep in mind that a Test Email will always be sent only to the default registered email address of your account, and as it is a test, the tags will appear unchanged - there are no values entered on the form that would replace them.
If you need to test out the email functionality of your form, you must instead make an actual form submission from your form directly:
