Dear sirs, on this url
I inserted a form (about 2 years ago).
Now i get the "form quota" alert, which frankly I find quite surprising, because i got TWO submissions this month.
Moreover, I can't find the form on my account.

The form should be fine now, there was a general issue that prevented the reset of the monthly counter in the account users.
