Why is my account suspended?

I tried to log into my account and it say auto suspend under the password field. 

After further checking your form, I have found out that some of your forms are collecting login credentials from respondents.

Please be reminded that we do not encourage our users to collect login credentials through our forms as it violates our terms. Also, we have an anti-phishing tool that scans and detects forms suspected of any phishing activities.

I have deleted the fields from your form that caused your account to be suspended so I can reactivate your account. Your account is now active and to avoid this from happening again in the future, please refrain from collecting sensitive information through your forms.
