I made several tests and I noticed that your stand alone form is also not fully responsive.
To resolve your issue first check Make This Form Responsive option on Form Designer page.
After that expand your "Patient Birth Date" field. Turn OFF shrink option.
You will also need to Top Align labels of your "Insurance Company Phone" fields.
