How do I remove Header from template

    There is an unseen header in my Wedding Questionnaire that appears as Heather Nan Photography in the submission.   I do not see this anywhere in the Build for the form.  I can't use this is other photographers name is on it.   How do you remove it?

    I've checked this form on your account https://form.jotform.com/61234506094148 as well as all the email history and did not find the header you're mentioning in the email logs, in the given form I cannot find such header nor in the email notification either. 

    Could you please let us know if you were able to remove the header? 

    If it's still there, please provide us the link to your form and we will be glad to further check it.

    It's still there... I think it's a glitch.
    I used a free theme. Made it my own. When a client fills it out. It comes back to me with the theme designers business on it.
    I suppose if it's not seen on the clients side it's not bad but I print these for my files.

    You could try deleting the current email notifications or auto-responders and then adding them once again to your form, his way you will get the fresh data and will get rid of the info from the old template.
