Were you trying to add image/cover logo to your form or you were not able to use the upload field on the form? Please clarify what exactly was the issue.

If you want to add logo/image on your form, you can follow the steps described here

    ---

I did not find an upload field on your form, so I think that is not the issue.

Let us know if issue remains.

    Thanks.

I have tried it as a header and as an image. I am going to use a google
form as it seems that if you can't even upload an image without a glitch
then I am not able to trust that any other functions will work.

Sorry for the confusion. Just like what my colleague said, you can insert your logo through the "Heading" or via the "Image" tool.

In Form Builder V4, click the heading field and then click the "Properties" button. After that, go to the "Heading Image" tab and click the "Choose a File" button. Click the "Upload File" button to upload your image.

Under the Basic tab, you can use the "Image" tool. Once you inserted it, the Image wizard will appear on the right.

If you're having issues, please let us know so that we can assist you properly. As much as possible, please provide more details about the problem. You can also provide us a screenshot of the issue.

    Hope that helps. Thank you.
