Quiz results not working

I have set up the email in quizform and it does not show the quiz results in the email.  I have it set up on two different forms and it works on one but not the other?  I thought I set them up exactly the same however not results in quiz two.  What could be causing this?

Can you please indicate what is the form in which it is not showing the results in the email notification?
